Engine flush was amazingly clean. I expected it to be darker. Fresh oil and I have 40 psi at idle, which is right in the middle of the gauge. Lifter is still noisy, so I'll need to keep a watch on that. Two years ago, it would quiet up after driving for 5 minutes. Hopefully it'll do the same this time. Plus, I'll go back to full synthetic after I run with this oil for a bit. That might help. I used dino oil for this round since I was flushing it.

I had an old IHC 345 with noisy lifters. I'd toss in a qt of ATF 100 miles before an oil change.
Lifter tick went away after a couple cycles of that.
Is your PCV system functioning ? You really seem to have an unusually bad case of sludge for one of those engines.

I think the sludge is because it's running rich. I don't have any codes, but I think my coolant temp sensor is bad because when I fire it up on cold startup, it idles like normal instead of idling high and then dropping down. I have a new sensor on my bench, but haven't gotten to it yet as I've been chasing other things and just been busy until now. It's next on my list as a "to do" and will probably do the water pump at the same time as it's weeping. Again, just trying to organize the top items first.

Those year 4.3s don't have adjustable rocker arms, so if the lifter is weak/failing, it would make noise. Mine (1999) has rocker tapping at cold start, but goes away with oil pressure and warming up. The offending rocker should be a bit looser than the rest.
