Of course this only applies to states that have them, but I think some people would pay a little extra for a mobile state inspector instead of having to drive to a garage and wait.
I imagine the emissions equipment would be the biggest upfront cost.

Tow companies, like semi companies, are full of meth and will do anything to keep workers on. Unless you're in a position to take advantage of a bad scenario (i.e. you know many and they nearly unionized over a bad workplace while a contract is up or something) I'd say no.
Don't forget the whole "24 hours / 7 days a week" thing, too.
