We have a 2013 Lexus GX460 with the 4.6 v8 and about 160k miles. It runs perfectly other than low fuel mileage in the mid 16s. It gets regular synthetic oil changes and all that- but I'm wondering if it's worth it to change the plugs. Normally I would, but this thing runs great already. I seriously doubt it's ever been done.

I am no expert but I would check the owners manual for the interval recommended by Lexus to change the spark plugs and follow that. If it's already running great that's good but change them anyway. It's all about prevention and maintenance.
Probably Iridium plugs if they're still doing well this far in, but better to change them now, before it eats a coil or starts badly misfiring when you're away from home...
Just because it's *currently* running well doesn't mean you should ignore the maintenance schedule. Lots of people learn that the hard way with timing belts.
I believe it's a 125k interval. I suppose I could tear in this weekend.
Wow, yours gets 16mpg!!!!?!?!? Dont put a roof rack, led light bar, and 265/65/18 ko2s on it, mine gets 13.

Based on it's 15 city, 20 highway, and 17 combined official rating, I'd only say it's significantly 'low' if pretty much all steady-state highway miles. So while I wouldn't go in expecting much/any improvement, I also see no reason not to replace the coils and plugs anyway and be pleasantly suprprised if there is.
Yes, you should replace the plugs. They're most likely iridium and when those wear what happens is that the iridium tips come off opening up the gap dramatically. If you don't get a misfire right away do do soon after when the larger gap takes out the coil.
