So I'm trying to get hired on by a transportation company. They want my DOT Physical long form. Ok, I had a DOT Physical when I started at the company I currently work for. I had gotten the certificate and card, so I called the Occupational Health facility to acquire the long form. They proceed to tell me since the company I work for paid for it, it's theirs and I can't get a copy. Well that doesn't seem right.
So, I call my HR department. They proceed to tell me that they own that information and they have no obligation to give it to me (they actually told me that in a much more rude way).
So now, even though I have a DOT Physical that's good for two years, I can't use it and have to drop $100 on a new physical because there's no place cheaper close by.
They're not wrong. They paid for that information. Why should they subsidize someone else?
Just thinking- wouldn't the necessary information be part of your medical record? You should be able to obtain that for free.

I understand that they paid for it, but I thought that I would be able to have that information since it is my medical information, this is kind of a rude awakening in some sense.
This is just another thing I have to go get done in what limited time I have.
I just ran this by SWMBO (she's a medical type).
Just call the place and request a copy of your medical records, they have to give them to you. I guess it might take them a couple days though.
Well, isn't the DOT someone you can pay for a copy from? I assume that form was sent to the DOT at some point.

You are asking the wrong people.
Your employer has no obligation to share their personel info, and state agencies may not even have it. GRMers are not medical providers. You need to be asking the medical provider for your medical records.
Your DOT physical card should have the examiner's phone number. Call them and request a copy.

We do our physicals in house and I believe the DOT only sees a copy of they perform an audit or if the want to see it post accident. We also don't give anyone copies of them. We paid for it and it's ours.

If you re-read my original post, I never asked for GRMs help, I was ranting. I don't know where you are seeing that I'm asking the folks here to provide me with my Physical information.
As I already stated called the Occupational Health Facility, which is run by the local hospital. They will not give me the DOT Long Form information/copies/records I need, because I didn't pay for it. I confirmed this with two different employees. I only received the Certificate and Card.

I never told them I was leaving I just played dumb and said I was told that I was supposed to get a copy and never did.
Because you have a right to your medical record, doesn't mean it is cheap or easy to obtain. I wanted to get a complete history of what I had done and when, but and every doctor I asked said it would take weeks to process, and they would have to manually xerox each page of my history, and it would be $5 a page and blah blah blah. All I want to know is the date I had my appendectomy, my toe surgery, my last tetanus shot, etc. Impossible to get the highlights without a lot of time, effort and money.

I did not, but they made it very clear that because I didn't pay for it, they can't give me the information.

So my thought would be to tell them you have the card from the last long firm physical and once you receive a written offer of employment you will be happy to get it done agin so they can see the paperwork. Tell them their written offer of employment can be conditional on producing the document within x days of receiving the offer.

That's not stopping them from hiring me. It's just become inconvenient. I am going tomorrow to get a new physical done, because they will need it before I start.
