There's more than one way to lose a wheel. These lugs were properly tightened when the studs they were on sheared off:

That's a good point. Having the door open was probably the best thing to happen there. At least it wasn't glass and a tire to the face.
I had a 1970 Chevy pickup with soft aluminum centerline wheels on it, and I drove along the side of a hill and snapped half the studs off of one wheel (heard a noise but it was dark and didn't notice). The only reason I didn't likely lose a wheel on the freeway is the timing gear broke a short time later, after that I never really got it working again and sold it.

Wow. You can see the wheel flex at the outer edges of the spokes before the lugs shear.

So this is the front left wheel of the Toyota Corolla I rented from Alamo.
I noticed a slight noise that kept getting louder as the miles went by, finally stopped ot se wtf was going, took the hubcap off and was greeted with this.
So yeah, always, always check the lugs on a rental car.
After the first time you never forget the feeling and sound of a wheel so loose it starts to wobble and hammer the studs off. When I pulled over and caught mine it had snapped 2/5, don't install wheels when tired.
Lost a wheel off of trailer once. It's a bad feeling when your wheel passes you on the wrong side of the road.
Lost right rear wheel on the front strait at NHIS. It passed me at start finish and rode the concrete wall all the way around to t3. Turns out the steel wheel cracked just out side of the lug nuts and the center separated from the rest of the wheel. No pix as this was years before cell phones or even digital cameras.
The hub on the right rear of my ministock Neon broke in turn one a couple of years ago. Wheel went down under the car, grabbed and jacked the car in the air high enough that the front tire blew when it landed and hit the track.
Didn't know whether to E36 M3 or wind my watch for a second or two.
