I'd say the difference with those is that you're only insulting the person you're talking to, not any group of people (with the possible exception of "Bob Costas"). Using "fag" only works with the implication that it's wrong to be gay.
Now, I will refer to 80's hair metal as gay because tight pants, a vest with no shirt, and permed blond hair is incredibly homosexual.
