So let's say that you built a car hauler from, oh I don't know, the front half of an Audi 5000 spliced into the A-frame of an open deck trailer, and the dual rear axles replaced with a single extra-wide axle flipped upside-down to work with the reverse-rotation Audi driveshafts so you can have AWD to maybe prevent getting stuck WHEN the thing sinks ten inches into the ground between uses.

How do you title and register it? How do you insure it?

Or should I just shut up and go back to thinking about going Joad Family on the first ratty old hearse I can find?

It would probably just be titled and registered as an Audi 5000 since that is the base vehicle. Now if it was a completely custom body and chassis only using running gear from said Audi, that would probably be a kit car. It probably also depends on your local DMV.

I did not stay at a Holiday Inn Express last night so don't quote me on any of that.

That said, do tell more!

I would be more concerned with explaining it to my insurance co than to the state bureaucracy.

I think I'd register the Audi, THEN start building.

I registered my jet ski trailer as a jet ski trailer, then I built a camper on the frame.

Yeah, good luck explaining how you safely turned an old Audi into a ramp truck. Either voluntarily at it's creation, or involuntarily later on should it become involved in an accident of any sort.
