How does the IRS go about determining if you have had insurance throughout the year to determine if you get fined or not?
My wife is freaking out thinking that they are going to fine us because it does not show anything on our W-2.

It's the IRS, there's a form for that . It looks like your health insurance company may send you a 1095 B stating who had coverage for all months in 2015.
