Need Help Finding Obsolete Home Light Switch

I have to replace a light switch that has 3 toggle switches in which each operates on a horizontal plain and all three are stacked vertically. I need the switch and a switch plate. I don't want to do surgery on the wall. I have not been able to find this kind of switch at any of the typical sources. Any ideas?

What about buying the mounting plate and the switches separately? Like this:
