You have to have a bank account associated, then transfer funds.
Money is "stored" in paypal until you tell them what to do with it.

Depends. You can have them do that. If you don't want to give them bank account info you can (or could, it's been a while) have them mail you a check instead. This will take longer, and there may be an additional fee, I'm not sure.
Do NOT use your normal bank account! PayPal's User Agreement gives them express permission to pull funds from your account and lock you out of it with no warning. Yes, your actual bank account at the bank.
You need to setup some crappy free checking with like $5 in it and use that as your PayPal-connected account. They will not mail you a check anymore, you have to have an account with them. Further, if you actually desire your money within the next 2 weeks, they will also want your address, phone number, SSN, and a credit card.
