Surprisingly, the bolts are still secure. The bolts are 13 inches from the back wall so a standard toilet will fit. However , The drain pipe is not completely level with the floor. It appears there was a half inch lip of the pipe protruding from the floor which has now had pieces of it broken off.   I'm assuming I can't put a wax ring over top of this or can I? Or do I install a plastic fitting that goes inside the pipe and has been secured to the floor?
 

I'd appreciate any help? Thanks!

I had a similar issue in our back apartment and used one of the plastic inserts.

The better wax rings have rubber collars on the bottom side.  I'd grind off the rough cast iron edges.  Then I'd use one of these and call it a day:

 

Assuming there is not damage to the pipe lower than that.  I wouldn't leave raw concrete exposed to sewage.  It absorbs.
