So #1 daughter's bathroom has a steam shower that has a standard bathtub type spout  so you can check the temperature with your foot before you get in. The issue is the piping inside the wall has come loose so the spout is loose. No matter how much I try to set it against the wall it shifts from side to side. This means the spout is always coming loose from the wall. The shower is tiled with something that apparently is no longer available. The wall behind it is the backsplash in our kitchen. If I break either it'll give SWMBO an excuse to redo the entire room.

My question is, if I take the spout off can I fill the space with something like PL Premium to secure the spout. Any othe suggestions?

Pull the spout, poke a small hole next to the pipe,  blow Great Stuff insulation into the wall cavity through the small hole.  Caulk hole, and gap around pipe.

Reinstall spout, but caulk it to the wall with PL Premium. Let it set for 24 hours before using.

As a professional, I kinda hate to admit how often I have glued stuff together with Great Stuff!

Like your doctor doesn't just search Web-Md half the time when answering questions.
